Athletes Forms

Which athlete, representing the Tashkent International School in any kind of tournament or competition is required to fill out the two forms, which are the Athlete Code Of Conduct and the Medical Form. The Athlete Code Of Conduct form is there as a contract between the school and the athlete, in order to set out some guidelines of being in the team and the guidelines, by which the athlete should act in different situations. On the other hand, The Medical Form is there in case of an emergency, which will give the information to the coaches and the Athletic Director and help to get a better picture of the issue and the background of the health issues. Part of the medical form is also a parent consent form, which must be filled out specifically by the parents, where your parents give the permission to the coaches or the athletic director to give the first aid to the athlete and etc.
